I have been reading the new 'Death of the Family' comic books and was wondering how and when did he get his face cut off? (I just read Batman none of the surrounding bat family(Batgirl, Nightwing etc.)).

It was cut off by the Dollmaker in Detective Comics #1 (2011 volume). I think that issue is 99 cents on Comixology if you want to buy it, although I thought it was REALLY bad.
