I am pretty sure they did in his last limited series where they revealed his past and the origin of his powers but i am going on memory and could be wrong.
Quite a few times. In Moon KNight and his mini.
Several times, probably. In the UDON mini, he uses a consistent appearance in his "public" life that matches the flashbacks of his childhood in one issue.
Of course, that story is directly contradicted by the "amnesiac SHIELD Agent" mini, which apparently also showed him unmasked.
To be honest, his face isn't that big a mystery. The mask is less for secrecy, more for effect.

Several times but he is sort of ordinary looking without the mask and is known for it so it doesn't really matter.
Here's his face when Moon Knight ripped off his mask. Not sure if this is before or after Tasky wet himself.

He looks like a kid in that scan, not an experienced tactician...
One of the reasons I really hate Finch's work. He just can't do faces. It's almost like he taught himself by reading Liefeld stuff - noses, lips, excessive crosshatching, weird poses that break human physiology...

He almost looks old enough to be the Vulture in that Daredevil scan.

It's super annoying when artist's draw Taskmaster's mask like it's his real face. Such as an open skull mouth that leaves absolutely no possible room for a normal face underneath. Or the eyes drawn in a certain way. Happens all the time.
In his last mini series they showed his face of course there was no evidence that it wasn't his real face,
but he never said it was or wasn't so i guess you can interpret it both ways?
