No she has not. She was part of New Mutants, then X-Factor, then Excalibur, then she stayed on Muir Isle as part of no team. Then she taught at the school for a little bit but was not considered part of the X-Men (neither were Dani or Karma at the same time, in the same title). Then she joined X-Factor, where she's been since except for her stint as part of X-Force.
Not announced anywhere to my knowledge
But we don't know in how large of a capacity or if its just one arc. (Please point me to an interview or announcement if it says otherwise)

I think you forgot to place Iceman in the Astonishing line-up.
Thanks for the heads up. I was holding off b/c Liu said there would be roster changes, and Karma, Warbird, Wolverine, and Gambit are the only ones staying for sure and it's highly likely Dr. Reyes will stay too. That leaves Iceman and Northstar as the most likely ones to leave or take a break for a bit.
