I'm pretty sure it's as simple as this: he meant that scarecrow's gas was an example of fear being induced by a chemical or mind control technique, but Two Face's stare was an example of fear being a result of a real physical threat, like the one he was now posed with.

It might be "exagerating" instead of lying. I'm assuming it's unpleasant to stare at that eye, especially when you're tied up and facing likely death at his hands.
