That's a good point. However, the speculation at the moment is that he was controlling the robot body across the dimensional rift, and that link was severed by Tony. Peter was unmasked and killed shortly after that. So it stands to reason that there's a chance that Mysterio hasn't even been back to the UU since then and isn't even yet aware that Spider-Man is dead, let alone his secret identity.

Mephisto could already go there at any time in theory and he was responsible for the UU defenders.(we just presumed he was only the UU version when really beings involved with hell dimensions tend to be multiversal in influence) The UU already had some cameos in places like exiles, she-hulk, and that bad infinity gauntlet storyline with red hood Bendis wrote so it's always been a parallel dimension. The other villains you speak of don't really have any motivation to go there over any other random dimension anyway.
I dont like the idea of a crossover myself if only because Marvel said years ago that if they do a UU/616 crossover they've run out of ideas. Given that the main event in 616 right now involves a rehash of Rachel Summers with a different person badly told I suppose they probably' HAVE run out.
It's also entirely possible that he was in the 616 during spidey's public unmasking and went into the UU mid retcon so he knows something about OMD(at least the marriage component). There's alot of grey area to go over.

I just remembered, and correct me of I'm wrong, but after Ultimatum, during his online broadcast speech, didn't Mysterio say that the media has been calling him Mysterio and that although he wouldn't have chosen that name for himself that he likes it? So if he really is 616 Mysterio, what the hell?
I'm leaning more and more toward the Mysterio from Annual #3 and New World According to Peter Parker being the actual Ultimate Mysterio but the robot from Death of Spider-Man being an imposter controlled by 616 Mysterio. Although that also is iffy because a recap of all of his doings was shown during that arc and it included everything from the annual to DoSM... Albeit the recap was wrong, in that it showed the ending of the annual as taking place at the end of New World According... Maybe this previously pointed out continuity error could be a clue, or at the very least Bendis trying to fix it or explain it using the involvement of 616 Mysteio...
Bendis has some 'splaynin to do! ;P

It can easily be misdirection. By saying others have called him mysterio he sets his own stage name up.

While this is true, it seems an odd way of going about it. Why not just say "You can call me Mysterio" and be done with it? Something isn't adding up. Either Bendis lied on Twitter or he's using Mysterio like this because he can't think of anything else.
Also, that one speech appeared 3 times in 3 separate issues. Bendis wanted us to remember it. So the chances that he simply forgot about it are slim... So hopefully he explains it. Perhaps an issue will have flashbacks to all of his first appearances but delve deeper and show how him being 616 fits.

just showing them on a computer screen or in the background kind of thing. She hulk basically had it so that she had to convince some big all powerful entity to not destroy the 616 because it found the UU and was going to destroy the 616 so that UU could replace it. She convinced it that 616 was fun and shouldn't be destroyed in favor of the "new sexy wife" UU. I dont recall them actually showing the trial so much as She hulk saying she had to convince the entity to everyone else. It was pretty clearly the UU she was referencing as the "replacement."
In exiles they had a bunch of screens showing alternate realities and UU has shown on there a few times as an Easter egg.
Nothing so overt as an outright crossover but it's been established as being an alternate dimension for at least a decade.
actually now that you mention it i seem to remember a scene in a recent 616 avengers arc involving ultron depicted the ultimate universe as another dimension
