You'd think that Captain Britain of all people would know that the Queen is addressed as Majesty and that Royal Highness is used for other members of the Royal Family. Obviously he's been hanging around with those Avengers types too long

It's a queen-bot. As you can see, it's the union jack flying over Buckingham Palace NOT the royal standard, which indicates the queen isn't at home. Therefore, that can't actually be the Queen. Further proof maybe had that this isn't the Queen because she is wearing a particularly unflattering dress (the colour's good but the shape isn't, and this is the Queen, she doesn't dress badly and Hardman is a brilliant artist so it's not unintentional) and she describes her citizens as "your people" to Captain Britain. The real queen would use "our people". THEREFORE, we can only assume that MI-13 have a queenbot policy in place, that when the palace is under attack they place a fake queen in there so if worst happens, the baddy thinks they've got the real queen. the queenbot would probably explode and kill whoever was menacing her. Captain Britain clearly knows this and therefore does not use the address which should only be given to the one true queen.
