Rukia's thing was more comedy though. In fact, I'd bet that in Japanese culture, her unconscious ass floating to the top was at least 90% comedy, and her pic on the cover would be considered more fanservicey.

The manga doesn't often flaunt Orihime or the other hard bodied girls in the pages, but doing so on the covers is not unheard of in Bleach.

It's also possible that he used the process of elimination. He knew that Byakuya, Renji and Rukia had been brought along to be healed.

Byakuya and Renji bobbed up out of the water so it's logical that the third person bobbing would be Rukia.

More or less. In general, the Japanese traditionally liken women's bottoms to peaches, the peach in turn being a fertility symbol. This all goes way back.
