Also, both Blackbeard and Shanks look evil as shit in it. I doubt it means anything in Shanks' case though...eh, maybe.
Top right is Big Mom. Baggy eyelid, feminine eyelashes, and bloodshot eye.
Top left... I dunno. Could be Brownbeard because he's relevant to this arc, but that's a tenuous guess.
If it was Brownbeard I think that it'd be a bit more obvious and less hidden. Maybe it's one of the arms dealers.
I think that most of the people shown on the cover are characters that will be important in an upcoming story arc. Only Blackbeard and Shanks stick out from that idea since I doubt that they'll show up this soon.

Doflamingo has a crew.
Jinbei had a crew.
Moria had a crew and was making a new one.
Crocodile presumably had a crew before starting Baroque Works.
Kuma we know nothing about, but he was at least involved in the Revolutionaries before.
Mihawk is the only one that is totally independent. It kind of makes me wonder if he just sailed around in that little boat all by himself the entire time as a solo pirate. It's kind of telling that the only person we see him on friendly terms with was someone he fought against countless times, Shanks, who by his nature is just plain gregarious.
One theory I heard that I really liked was that Mihawk was supposed to represent what Zoro might have become if he had never met Luffy. Someone with no purpose, no friends, just perpetual boredom after achieving his title.
I wonder if he just never had anybody on his side, and that's what led him to become so strong, because he had to be strong enough to take on the Grand Line as just one person.
Whereas Zoro has become far stronger because of his relationships with his crewmates. And it was his love for them and need to get stronger for them that led him to humble himself and ask Mihawk to train him.
I'd bet the reason Mihawk agreed wasn't simply out of respect for that humility, or because he was amused by the irony of training his own challenger, or impressed with his willingness to go to any lengths achieve his ambition. That grit and determination was enough for Mihawk to spare him when they first met, but it's clear when they met again, Mihawk was disappointed in him.
So that wasn't the reason.
It was because Mihawk wished that when he was younger, he would have had friends like Zoro does. And he wanted to make sure that Zoro got strong enough to keep contributing as his crew's swordsman in the New World.
