...Didn't the "SHIELD is a secret head of HYDRA" gimmic get pulled way, way back in Nick Fury Vs. SHIELD? Jeez, just for once I'd like to see SHIELD presented as a 100% good-guy, kick all butt organization, with no internal squabbling or traitors.
If SHIELD's been run by Hydra since the beginning, why did it keep beating Hydra (it was crapping up major Hydra operations, that's above and beyond a for-the-cameras defeat) and why didn't Hydra take over the Earth years ago?
And why didn't it use its control to have all the superheroes in the 42 prison either murdered or cut off its return-access to Earth so everyone would be trapped in the Negative Zone?
And why didn't it use that easy access to the UN and every world government?
And etc etc etc...

It does however explain the thuggish behavior of SHIELD troops over the last few years, if not the amorality of Nick Fury himself and his habbit of using people (which now includes children for his private war, oh well, they are just lives after all and Fury is most likely as willing to throw them away if it achieves his goal) However if HYDRA or LEVIATHAN has been in charge than there is no way they don't have the list that Tony Stark is now trying desperately to hide. So that part of the story clashes with the rest of the mu.
But for this to work will take a massive amount of retconing, ignoring what has gone before or cherry picking things that will help the story and not hinder it. Characters will have to be retconned to fit into the storyline.
Sounds like typical modern day marvel. Might be a good story anyway though. I have the feeling that if you take this as a What-if story and don't think of it as fitting into the standard mu that was it might work and work well.
