i dont know why im saying this because i basically got bombed on for mentioning half of what i am going to mention now but The Sketchbook shows someone looking like aunt may and people are saying that its from her childhood memories but i highly doubt that. I seriously think that there are now many clues present that can lead us a decent conclusion and my conclusion is as follows:

it is actually May's mother who passes on but it deals with May's mother not wanting to stay in this condition, possible coma. Does Aunt May fall into a coma as well? that could be very well a possibility but we will have to wait and see.

Peter is able to face Morlun on more even ground and in an all ditch effort battle finally put Morlun in his grave.

The Avengers gather around and everyone celebrates. Except Jarvis because now May is scary with all her Spider DNA and the world returns to normal.

Okay so I got a little out of hand on that one. But I do think that Pete will have to donate an organ. Which I think is a great message for a comic to have. My Grandmother's life was saved because she got a heart transplant. We got like seven more years to spend with her... so I guess I am pro organ donation.

My theory is "Peter won't come out of this whole" is not physically whole, but emotionally - in order to defeat Morlun, Peter will have to do something so horrible, so unlike who he is, that it will leave great emotional scars and he will never be the same again.

in a much more morbid, pessimistic view - Peter could lose all his powers. Morlun succeeds in sucking out Spidey's powers but doesn't finish the job due to some sort of distarction and Pete's still alive... but powerless. So therefore, Peter must "Evolve" into something (spider-like, I assume) "Or Die"...Maybe he'll go back to that ancient cave where Ezekiel got his powers...

Check out the censored promo art in some of the Marvel books from the month before the Other started. 2 pager. One of them clearly shows the organ in question with a black strip over it. Looking at the upper part of the organ, which seems round at the bottom and thinner as it moves upward into vein like structures, I have to say it's his heart. My theory is he'll bond with the symbiote, "the other", to stay alive. Here's hoping.
