You know not nipping it in the bud when Bruce was young or trying to get him help
In fairness though batman only decided on the bat suit when he was an adult. Think he even tried to be a cop..so it may not of be as obvious to Alfred

Bruce should have undergone therapy.Even though he knew Leslie Thompkins i dont think she gave him any therapy.Probably romacing with Alfred. So indirectly its Alfreds fault as he didnt do anything.

An industrialist billionaire as a copper. Maybe he did advise him to get advice, but how do we know that Bruce spoke to Alfred about his inner torment, remember Aldred is more of a friend to Bruce so'll be more likely to support him in his decision. Alfred also suffered a pretty heavy loss as well, and may have wanted to impart his own brand of justice

Do we know he didn't have therapy?
I think we can infer that if he did, it was unsuccessful or an poor attempt. If there really is a therapist out there that knew all of Bruce's deepest and darkest secrets, I think he'd be a better known character like Leslie.
not sure, but he sure couldn't have helped much. with all his bickering and whining, all he seems to do is to 'enable' Bruce to accept his Batman persona.
I don't think he's as innocent as he sometimes seems.
