Have there been any deaths of HEROES or VILLAINS that have had a lasting effect on continuity? I mean deaths that are still felt today.

Death of Superman due to all the characters it introduced and the re-percussions of their actions.

I would think the Barry Allen death was pretty impactful.

Bart Allen's death made us not care anymore... Does that count?

"Was" being the operative word here.

Seconded, though he didn't stay dead, Superman's death & return gave us tons.

It didn't just impact the Superman titles, it impacted everything around it.

Could you imagine what Green Lantern comics would be like right now if Coast City was never destroyed? Cyborg Superman did that, and would never have been able to if he wasn't allowed to pose as the then dead Kal-El. Actually thinking about it, Superman's death may have had more of an impact on Green Lantern than even himself.

His death gave us Superboy, who became so popular that his own death & return had massive ripple effects of it's own.

Not really, because nobody is staying dead.
