A quick Black Mask Question

In Batman 387, we see Black Mask get his mask burnt onto his face. Instead of the skull like face we all know and love, his face simply looks like the mask he was wearing at the time. Like so:

My question is, did he appear like this ever again?

What's the first issue he shows up with a skull face?

Do they ever explain why his face changed to the shape of the mask to a skull?

Yeah, he wore this for a while. Pre Knightfall. During Moench's run with Jones. Maybe in Brubaker's Catwoman run is when we see him look like a skull for the first time. But I could be wrong...
