Okay, I've been looking for this for quite some time now, and I haven't been able to come up with anything definitive. Is there a list somewhere (complete or not) of which titles have been confirmed as taking place in Earth-616 reality?
For that matter, a list of titles from other Earths would be beneficial, too.
Thanks!

Pretty much everything is 616 unless labelled otherwise.

I'm not entirely sure that's accurate. I'm very far behind in my current reading (I'm currently in September of 2008), but I've run across quite a few things that don't seem to fit.
I'm assuming that all of the Marvel Adventures titles don't take place in 616. For some reason, possibly the lack of note of any 'current' events, I don't think that Invincible Iron Man (Vol. 2) takes place in 616. There was something else that I can't recall right now that had Dum Dum Dugan in it, but by that point certain events would make it impossible for him to be there.
Newuniversal doesn't take place in 616 does it? Or Squadron Supreme?
I'm just trying to keep it all straight, and try to stick to a chronological reading order. Especially as the events of Secret Invasion are unfolding (hey, I said I was behind in my reading, right? )

The only one i remember is the recent X-men Ghost Boxes. In 616, if i remember correctly, the most of the AXM are killed. I think Cyclops is the only one left alive.

Nopers, 616 is a fan thing, i guess, picked up from UK Marvel comics. No one in Marvel actually calls 616, 616.
Oh look at what wiki says.
Most references to Earth-616 appear in Marvel UK titles, in Excalibur, or in Marvel reference texts such as the guide to Alternate Universes (2005). In addition, there are a number of other references to Earth-616:
In Marvel 1602 #6 (March 2004), Uatu the Watcher refers to the universe as Earth-616.
In Marvel Knights 4 #15 (April 2005), Earth-616 is mentioned.
In Uncanny X-Men #462 (September 2005), an alteration in reality of Earth-616 causes trans-temporal devastation, and Saturnyne attempts to destroy that particular temporal continuity to "cauterize the wound".
The term has also appeared in Exiles (in, among others, the House of M tie-in issues) and is in regular use by the writers of Marvel's Official Handbooks for the simplicity of the term.

Agreed on its usage. I knew it was a 'fan' thing as well as an in-house reference (especially since 616 was just pulled out of the air at its inception).
The list of 31 titles above is helpful, but I'm guessing it's going to take a bit to get it as detailed as I would like.
However, I was under the impression that Astonishing X-Men did not take place on 616. At least at the beginning of Vol. 3 when Whedon was writing it. I've not read AXM from the beginning, only picking it up around issue 24 from March 2008. I don't recall where I heard that AXM wasn't cannon, but I'm assuming that from issue 25 it does take place in 616 and has become cannon from that point.
Looking at Wolverine Vol. 3 66 and 67 (haven't read past 67 yet), I'm wondering if it takes place in 616. The "Old Man Logan" arc seems to be a different future than what we know of 616.
Guess I'm going to have my work cut out for me trying to figure out the minis and one-shots, as well as the older issues. For example, when Amazing Spider-Man Vol. 1 became Amazing Spider-Man Vol. 2 in 1999, I'm assuming that it was still taking place in 616. Again, it's another section of comic history that I haven't read.
Thanks for the starting point, though. And if anyone wants to chime in with other titles as they come across them, it would be greatly appreciated.

The AXM Ghost Boxes was a two issue special following the recent story in AXM about alternate realities and one of the realities shown there is referred to as 616.
I never read AXM until this last relaunch so........i have no idea where in continuity that stuff is.

"Old Man Logan" is a possible alternate future. SQUADRON SUPREME and NEWUNIVERSAL are alternate universes.
ASTONISHING X-MEN is entirely in continuity. Barring time travel stories and one-offs like "Old Man Logan," titles don't generally jump in and out of continuity.
CaptainCanada has the right of it. Assume everything's in continuity unless it obviously isn't.

The Official Handbook of the Marvel Universe Hardcovers ran a master list of alternate Earths that numbered over 1,100 but it covered everything from the comics, novels, TV shows, cartoons and movies. I't is a good place for which comics took place on which Earths. I remember one of the early Handbook had a list of titles that took place on Earth-616 up to the point it came out.
For internet sources
The Master List Guide http://www.marvunapp.com/master/mastguid.htm This pretty much covers all the marvel characters and under the list for Earths this pretty much covers all those from the hardcovers with a few minor exceptions as well as some that didn't make the Handbooks.
The Appendix to the handbook of the Marvel Universe http://www.marvunapp.com/Appendix/ covers numerous Marvel characters as well as Alternate Realites and covers what books took place there.
The Marvel Chronology Project http://www.chronologyproject.com/phpbb2/index.php is a good place they keep track of the books that take place in the Earth-616 reality as well as some alternate realities.

Why is it called 616? Is that the area code for New York City or something?

Nope. New York City is mostly known for 212 as the area code. This has nothing to do with the designation 616, though. I forget who actually gave it the number, but basically, they just pulled the number out of the air. Kind of even in an off hand way, 'We could call it anything at all. Doesn't matter. I mean we could call it Earth-616 for all I care.' (Not a quote, just my memory of an article I read a long while back.
Also, many thanks for the links you all have provided. And the lists of upcoming books? Phenomenal! Thanks gang!

It's just the random reference number the guys who over see Marvel's multiverse gave it. Every alternate universe has a reference number. Kind of like a barcode.
