Had to be early 90s/late 80s.
I'll try to list as many details as I can recollect.

It's not "Going Sane", but similar.
It was in an annual book or something similar.
Featured Joker getting knocked over the head and going into a coma where he wakes up as a normal guy in a radically-different Gotham. The "Batman" and "Joker" of this Gotham are like elemental demons terrorizing the city. He sets out to confront his "usurper", and that's about as far as I can recollect.

Finer details:
Started out with Joker pulling off a job and sporting a really terrible-looking mullet. Gets knocked over the head and wakes up next to wife, he's window washer or something. Gotham is... I don't want to say steampunk, but it's a mesh of different eras. Joker resolves to replace his counterpart and puts on some clumsy makeup before facing what's like a death-metal clown version of himself, then Batman crashes in like some sort of gargoyle monster. I think that's where part 1 ended.

Could you be referring to the Dark Genesis three-parter that ran through Detective Comics? That's the only story I can think of featuring a demon Batman, but I'm not clear on the details of that story.

I think it's a two-parter from Showcase '94 issues 1 and 2, written by James Robinson.

Yes, it is that one. The first two issues of Showcase 94, by James Robinson. I cannot recall the interior artist, but the covers were done by Kevin O'Neill.

Great art by Christian Alamy - a bit of Brian Bolland and Simon Bisley.
