1. How did the Kingpin go blind?

2. When did the Kingpin learn Matt Murdock is Daredevil?

3. When did Foggy learn Matt Murdock is Daredevil?

1. A guywhosenameiforgot shot Kingpin. He managed to recover but the cost were his eyes.

Kingpin was shot in the face and blinded by Echo in DAREDEVIL v2 #15 (the end of "Parts of a Hole") by Mack and Quesada.

Kingpin learned Matt's secret identity in DAREDEVIL v1 #227 (the beginning of "Born Again") by Miller and Mazzuchelli.

Foggy learned Matt's secret identity in DAREDEVIL v1 #347-348 (the middle of "Divine Comedy") by DeMatties and Nord.
