I've been wanting to get into the Hulk for a while, but I'm not sure of where I should start. I was wondering if anyone could recommend me some stuff as a starting point. I'm looking for something recent, with a gritty, more realistic approach (like MAX), and with an emphasis on the psychological side of the character. However, anything would be good. Thanks.

Banner, by Azzarello.
Really grim and gritty Hulk.
Also Future Imperfect is pretty gritty.

Bruce Jones' run started off that way, but . . . well, that'd probably be a bad place to start.
I'll suggest GIANT-SIZE HULK, the recent special issue. It has two new stories and reprints HULK: THE END. Banner's fractured, tortured psyche is front and center in two of the three.

Does the Hulk actually care when he ends up putting someone in the hospital like he did the Thing?

Probably not.

If you want the best psychological tales for the Hulk, delve into the recent Peter David: Hulk Visionaries of reprint trades.
No, these stories are not recently made. But are the best psychological dramas the Hulk ever had. Especially issues #468-479. Those issues are the climax of all the soul-searching going on between Bruce Banner and the Hulk. The Visionaries haven't reached that point yet, but they damn well should. A vintage time for the Hulk.
I really can't recommend Peter David's Hulk run enough. In terms of the deep relationship between Banner and the monster, it gets no better than these stories.

I wholeheartedly second this opinion!
