Planet Hulk itself consists of Incedible Hulk #92-105.
There was also a four issue lead-in story about how Hulk ended up in space. That was in Incredible Hulk #88-91.
There is also New Avengers: Illuminati (One-Shot). Not neccassary but gives a little more back story.
And lastly I believe there is a one-shot Planet Hulk Special due sometime soon that has some bio's and Giant-Sized Hulk which has a Banner On Planet Hulk story.

The last issue of the first real part is out now so you can read it and have a ending to it and see if you want to continue getting it or not. I actually just read all of mine not but 1 hour ago (I dont read stories till I have them all)
To get up to speed this is all you need.
Incredible hulk# 88, 89, 90, 91. You really should read these first as they are the springboard for what happens next and will fill you in on all the details and while its a bit thin its a interesting setup. When I read it I think I actually said outloud "Those dirty mother*******!"
Incredible hulk# 92, 93, 94, 95. This is the actual story and has a ending in 91so its safe to read if your like me.
I had my doubts but after reading them all I did like it, I actually had some emotions towards the charcters in the comic in the sense I was pissed at them hehe.

Including Giant Size Hulk is kind of weird.
Couple points from a recent Peter David interview -
Peter David wrote the issue because of an editoral request for a Hulk story to tie to the old Champions team.
He didn't write it as a Planet Hulk tie in, apparently it will become evident how it relates to the planet hulk stuff at the end of the Planet Hulk story.
I'm loving the Hulk right now, but an issue that is an editoral mandate forced into a storyline doesn't seem like something I want to read. I'm into the Planet Hulk story for the run, but I'll pass on Giant Size until I read some reviews.

You do realize that David doesn't write the only story in Giant Size, right? There's also a Greg Pak story about Banner on Sakaar.

Actually I didn't.
But I was under the impression that the Champions story would some how eventually link to the Planet Hulk storyline.

When did Hulk get smart? Did I miss something? When they sent him out to space,he said he would get them back. Is Bruce Banner in control? and if so when did this happen?

I think it does, or at least links to post-Planet Hulk happenings.
But the Pak story links directly to the Planet Hulk event -- so skipping Giant Size because of a questionable link between PAD's Champions story and Planet Hulk will cause you to miss out on a story that actually does have an impact on Planet Hulk.

When is a good question. I say read the issues preceeding Planet Hulk. From there you see the Hulk slowly get smarter and by the time he crashes on the alien planet he is the Grayvage Hulk ( Mr Fixit's personality mixed with Green Hulks skin). It appears Banner has lost control since Hulk has yet to revert back to human.
