She took the little girl form so Thanos would be less comfortable with her, as she said,
"Lesson learned, Titan. I am Death. I am NOT comfort."
Also of note, she's actually speaking to him, which is a major step up in their relationship. Earlier, she wouldn't speak to him because he was so many steps lower on the cosmic ladder, and when he held the Gauntlet, she would not speak because he was superior to her.
She has spoken to him once before, at the end of The End before he recreated the universe, and she has spoken to one of the Thanosi, possibly to stop whatever crisis it had caused (Cosmic Odyssey Thanos was so far out of character it's generally presumed it was a Thanosi), but not so to Thanos himself.

She did? :confused: I thought she just kissed him and then left. At least that's what I remember but I could be wrong.

I thought they did? Or maybe I just misread somewhere, but I thought Death basically threw herself at Thanos and he brushed her off or something?
But yeah, the little girl thing -is- disturbing, but let's be honest....does she -really- think something like that is going to make someone like Thanos think twice? :P It's probably not even a tenth as discomfiting for Thanos as it is for people reading about him.
But speaking of not being caught up: Who are Thanos' two "sidekicks"? The little fairy-thing (Skreet?) and the "Fallen One"?
