I thought he was sealed away safe as houses inside the Rock of Eternity. But that's clearly him right below the panel of Alex Luthor looking at his severed finger.

Maybe there was a shape-changing alien that took the form of Captain Marvel to temporarily confuse a few of the heroes. Without the issue handy, I don't really know how he was acting, but I'll just assume that he was floating through the air.
A better question is why all those in space that were part of a different Earth (like Alan Scott) were not affected by Luthor's device.

Actually When the worlds got seperated, the marvel family got shunted off into Earth S. There is nothing saying that he is trapped in the rock on Earth S.
(In fact from what I heard somewhere, way back pre-crisis, Shazam had been dead for years. His ghost was the advisor of Cap.)

If it's truely a creator's mistake, then call it what it is. Having the fans do their work for them breeds a bunch of lazy writers, artists and editors.

I was re-reading the issue today and kind of wondered, if Deadman wasn't inhabiting a body, can he really hold someone's hands?

He hanging out with almost every magic user in the world at freakin Stonehenge. I'm sure someone there knows a spell that can enable Boston Brand to hold hands, either that or maybe one of the mgic users there has part of themeselves attuned with the spiritual plane so they can hold Deadman's hand. Or Deadman is possessing one of the magic users while holding hands and exited the bosy when the Spectre killed Star Sapphire.
Just pick whichever one of the above explanations works for you.

The Rock of Eternity is the center of space and time. So that Captain Marvel is either a Captain Marvel of the past (he sent himself on that mission) or the future (Captain Marvel just watched himself do the job, so he knows exactly what to do!

Actually, it was only hinted that he went into the RoE...
We don't actually know if he volunteered to be the next "Shazam" yet or not...

Captain Marvel was last seen at the Rock of Eternity, the very center of Time and Space. Cap can go anywhere and anywhen from there.
