Super strength, durability, speed, stamina and rapid healing. He was also made out to be quite knowledgeable in various fighting arts and tactics.
I can't remember if he's capable of manipulating the Astro Force without his harness, which he almost always uses anyway.

Not in a fully literal way. Without Motherbox calming him, he can go into a sort of rage mode.

In the Jack Kirby stories he did something (I think the Motherbox did it) to change his face from his natural ugly look to a prettier one.
These days that isn't mentioned and he seems to be somewhere half way between

In the original Series Orion's Motherbox gave him the appearance of someone of New Genesis origin. He is Darkseids son so naturally he isnt the most handsome. Also the Motherbox would help him control his rage.

As far as levels of power he's a match for superman and with typical kirby hyperbolie his blasts can shatter mountains/continents/planets.

During the Simonson run, he solved the mystery of the Anti-Life Equation, and then wished he hadn't. He also has a harness connected to foot flight disks that focus his Astro Force into a better controled power. I think the best suggestion I can give, is to check out wikkipedia.com. Everything listed previously is accurate, but I'm not sure how much more there is.

The "other form" you've heard about has been played with by many writers.
Evanier showed Orion transforming into an Eternity-like being when he took his "true form"...His body became an online of the cosmos and what not, a cosmic storm rose from his very presence on earth, etc...this is what happened when he got angry.
Then Pollack showed that the true form of the New Gods was gigantic...that the Mother Box and Boom Tube adjusted their size so that when they appeared in the "mortal" universe they were human size...when planets were shown next to Orion in his "true" form they were about the size of marbles.
Then Byrne played with the idea in his JK4W. When the Forever People confronted a mob mad from the pain of a dying Source, they showed their "true" forms, beings of pure energy...their very presence radiated life and hope and love and calmed the entire mob down just by looking at them.
Simonson was going to, but never had the chance, show Orion take on his "true" form as the God of War, sort of like the 'aspects' in Zelazny's "Lord of Light" novel. He felt that, though it could be detrimental to the universe itself, when necessarily Orion could truly become the manifestation of War. In this form he would have been capable of destroying Galaxies and he would have gone up against a threat from the Old Gods.
